HELP!! WHY THE MISFIRE? WHATS WRONG? HD

My 04 Chevy 2500 with the 6.0 engine. Truck has new Cam sensor, new Crack sensor, new throttle body, new plugs, new wires, new MAP sensor, new Mass air flow sensor, and new coils. this is me going from idle to wide open. And for some reason it revs then around 4000Rpm(Give or take a few) It pauses then continues on up to regular rev limit. It seems to throw the missfire code everytime it pauses during the rev.
