Действие игры происходит в постапокалиптическом будущем на территории бывших Соединенных Штатов Америки спустя двадцать лет после глобальной пандемии, вызванной опасно мутировавшим грибком кордицепс. В течение игры её герои, контрабандист Джоэл и девочка-подросток Элли, пытаются пересечь континент, сталкиваясь в обезлюдевших городах с различными опасностями — охотниками, каннибалами и обезумевшими жертвами кордицепса, напоминающими зомби.
